On or about Tuesday, February 11, 2014 at 209-35 Northern Boulevard, Bayside, NY 11361    An employee was observed exposed to a ten (10) foot fall hazard while performing a welding operation to a construction site staircase standing on top of a not fully planked scaffold working platform erected directly above an open staircase.
