29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread:  JOB SITE: 4855 Baumgartner Road, Saint Louis, MO 63129, at this facility, located in the production area, stair rail leading up the Concrete Mixer control room did not consist of an intermediate rail.  The workers were exposed to a fall hazard.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated;  the person certifying that the evaluation has been performed;  the date(s) of the hazard assessment;  and which identifies the document as a certification of hazard assessment:   JOB SITE: 4855 Baumgartner Road, Saint Louis, MO 63129, on or about 2/25/2014 at this facility, in the production area, the employer failed to identify a fall hazard with a missing mid rail on the stairway leading the concrete mixing control room during their monthly Safety Inspection Checklist.  The workers were exposed to a potential fall hazard.
