29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:     On or about February 27, 2014, during the hoisting and unloading of materials during construction, the employer did not ensure employees were protected from fall hazards ranging from 12 to 30 feet, by ensuring a fall protection system system/device was being used.
