29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, Rig #9. The employer did not assess and determine what personal protective equipment was needed when handling barite. Employees were not provided chemical safety goggles or chemical gloves when handling barite. Employees were exposed to health and safety hazards.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:    On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, Rig #9. The employer did not adequately train the employees to recognize hazards when working with barite and utilize the proper personal protective equipment such as chemical safety goggles and chemical protective gloves as is required in the Safety Data Sheet (SDS) of barite. The employees were exposed to health and safety hazards.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:    On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, Rig #9. Two employees who had facial hair were wearing particulate respirators, the beards may have prevented a good seal. The employees were potentially exposed to respiratory hazards.

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:    On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, Rig #9. Two employees were mixing barite and were using tight fitting respirators to protect themselves. The employer failed to fit test the employees prior to initial use of the respirator or whenever a different respirator was used. Employees were exposed to respiratory problems.
