29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: On or about February 27, 2014 and at times prior thereto, employees were working at a rig site, Rig #9. The pit area was in disarray with 55 gallon drums of chemicals blocking some of the passageways and pallets with supplies as well. There was also an abundance of water on the ground in some areas causing the passageways to be extremely muddy and slippery. This condition exposed employees to slip, trip and fall hazards.

29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, Rig #9. The stairway landing leading up to the rig floor from the east end was sticking up. This created a tripping hazard for the employees.

Two employees were mixing barite and were using tight fitting respirators to protect themselves. The employer failed to fit test the employees prior to initial use of the respirator or whenever a different respirator was used. Employees were exposed to respiratory problems.
