29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) South side of Millet Plastics Plant, Lebanon Pennsylvania - An employee worked in an 8 foot deep trench that was not constructed in accordance with the requirements of 29 CFR 1926 sub part P "excavations". The employees could not explain how to classify soil and select the appropriate benching techniques for the trench. An employee was exposed to injuries from trench collapse on or about March 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) South side of Millet Plastics Plant, Lebanon Pennsylvania - The spoils pile was not kept back from the edge of the trench at least 2 feet. An employee was exposed to injuries from material falling or rolling into the trench on or about March 4, 2014. No abatement certification or documentation required.

The competent person did not inspect the excavation and adjacent areas and did not identify hazards which could lead to trench collapse. The employee was exposed to injuries from possible trench collapse on or about March 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
