29 CFR 1926.502(d)(16)(iii): A personal fall arrest system must limit an employee's free fall to not more than six feet (1.8 m), nor contact any lower level:  a. On or about March 04, 2014, at the above addressed jobsite, an employee conducting torch cutting operations was not protected from free falling more than six (6) feet, thereby exposing the employee to the hazards associated with falls.
