29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:      a) At 17 Municipal Road Klingerstown, PA 17941:  The employer did not conduct an annual inspection of the energy control  procedures (Lockout/Tagout). Employees were exposed to hazardous conditions such as electrical and crush-by hazards when conducting Lockout/Tagout on various equipment, for instance the Gasparini press brake, on or about March 6, 2014.                                                                                          Abatement certification is required for this violation.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a) At 17 Municipal Road Klingerstown, PA 17941: The authorized employee, in this case, the Maintenance person who conducted most of the lockout/tagout at the facility, did not receive LO/TO training and was exposed to hazardous energy on or about March 6, 2014.     Abatement certification is required for this violation.
