29 CFR 1910.1026(i)(1): The employer did not provide change rooms, in conformance with the Sanitation standard, 29 CFR 1910.141, for employees who were required to change their clothes to use protective clothing and equipment for chromium (VI) exposures: In Painting Department, employees working at Paint booth A and Paint Booth B are required to change into protective clothing to perform job tasks but were not provided with change rooms exposing them to cross contamination of street clothes with work clothes/PPE and hazards of exposure to Chromium (VI) through dermal contact.

29 CFR 1910.1026(j)(1)(i): The employer did not ensure that all surfaces were maintained as free as practicable of accumulations of Chromium (VI): (a) In Mix Room, handle on door presented residue of paint containing Chromium (VI) exposing employees to dermal contact hazard to Chromium (VI). (b) In Paint booth B, accumulation of Paint residue containing Chromium (VI) was present on floor surface exposing employees to respiratory hazards from potential inhalation of Chromium (VI).
