29 CFR 1910.178(l)(2)(ii):   Training did not consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  On 3/13/2014, Big St. Charles Motorsports, located at 3830 West Clay Street, St. Charles, MO, did not provide training for each forklift operator consisting of a combination of formal instruction, practical training and evaluation of the operator's performance in the workplace.  The employer did not have any documentation available to show their forklift operators had been trained.

29 CFR 1910.178(p)(1):  A powered industrial truck was found to be in need of repair, defective, or in any way unsafe.  The truck was not taken out of service until it had been restored to safe operating condition.  On 3/13/2014, employees working at Big St. Charles Motorsports located at 3830 West Clay Street, St. Charles, MO, were operating forklifts in need of repair, defective and unsafe.  The forklifts were not taken out of service until they were restored to safe operating condition.  The company utilizes the following forklifts during daily operations:  1.  Clark forklift, SN GPX-230-0117-8530, had a defective/inoperable horn and was missing a rear light. 2.  Mitsubishi, SN W4075, Forklifts had a defective/inoperable horn, broken rear light and broken fan near the operator.

29 CFR 1910.178(q)(7):  Industrial trucks were not examined before being placed in service.    Such examination was not made at least daily. Defects when found were not immediately reported and corrected.  On 3/13/2014, employees working at Big St. Charles Motorsports located at 3830 West Clay Street, St. Charles, MO, were operating two forklifts which had not been examined before being placed into service.  The forklifts were in need of repair, defective and unsafe. The following forklifts were not examined prior to being operated on site:   1.  Clark forklift, SN GPX-230-0117-8530, had a defective/inoperable horn and was missing a rear light.  2.  Mitsubishi, SN W4075, Forklifts had a defective/inoperable horn, broken rear light and broken fan near the operator.
