(b) On or about March 14, 2014, for employees exposed to caught-in and/or amputation hazards where grain storage bins were entered while the bin conveying auger was operated without a cover to prevent an employee from stepping into the opening above the auger and potentially contacting the moving parts of the auger at the railcar load out elevator, 500 Fifth Street NE, Lake Preston, South Dakota.
