OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to struck by hazards from falling loads:  On and before 3/19/14, employees of Gunville Trucking Inc. were exposed to struck by hazards while handling loads with an overhead crane and a wall mounted jib crane, both of which were not equipped with latches on the hoist hooks to prevent inadvertent disconnection of the load from the hooks.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   On 3/19/14, the power cord supplying the Milwaukee chain hoist in the diesel shop was missing the strain relief and bushing where the cord entered the housing of the hoist which put stress on the conductors and had pulled the ground wire loose from the electrical equipment.
