29 CFR 1926.451(e)(8):  Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:  Right front of building on metal roof of building:  On or about March 18, 2014, an employee were exposed to a fall greater than 6 feet to the concrete ground below, while accessing a metal roof from the scissor lift, by climbing over the guardrails of the scissor lift.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a)  Front of building, Genie Scissor lift, Model GS-1930, left side of roof:  On or about March 18, 2014, employees were exposed to a fall greater than 10 feet to the concrete ground below while using the scissor lift, without the access chain attached, and painting the exterior wall of the building.  b)  Front of building, Genie Scissor lift, Model GS-1930, right side of roof:  On or about March 18, 2014, employees were exposed to a fall greater than 10 feet to the concrete ground below while using the scissor lift, without the access chain attached, and painting the exterior wall of the building.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Front of building, Metal roof of building:  On or about March 18, 2014, an employee was exposed to a fall greater than 6 feet to the concrete ground below while working/standing on a metal roof without fall protection, and painting the exterior wall of the building.
