29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about March 25, 2014, employees working approximately 18 feet from the ground were not protected from falling.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about March 25, 2014, employees used a portable ladder to access the roof.  The side rails did not extend 3 feet above the landing exposing the employees to falling when ascending and descending the ladder.
