29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):  a) for the entrance areas of the Fruit Storage Bin Platforms in the Fruit Processing Building, as observed on or about 03/25/14.  In that some of the entrances were missing mid-rails exposing employees to a 17-foot fall inside the bins.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:  a) in that the stairs to the Fruit Storage Bin Platforms in the Fruit Processing Building were missing mid-rails, as observed on or about 03/25/14.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a) for employees exposed to fall hazards while performing "kick down" procedures at the Fruit Bins, as observed on or about 03/25/14. When employees enter the fruit bins they are exposed to 3-foot drop offs from several ramps.  The only means to prevent falls was to hang on to a rope while walking down the ramps.
