29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.   On or about April 1, 2014, in the door shop, a Seki-XL vertical mill did not have a guard, exposing employees to flying debris.    b.  On or about April 1, 2014, in the door shop, a Bridgeport vertical mill did not have a guard, exposing employees to an amputation hazard.    c.  On or about April 1, 2014, in blue steak shop, a Hardinge engine lathe that did not have a guard at the rotating parts, exposing employees to struck-by and flying debris hazards.    d.  On or about April 1, 2014, in blue steak shop, a Sharp engine lathe that did not have a guard at the rotating parts, exposing employees to struck-by hazards and flying debris.

On or about April 1, 2014, in the saw room, employees were using a Marvel bandsaw that was not guarded to keep operators hands away from the cutting edges, exposing employees to cuts and amputations.          b.  On or about April 1, 2014, in the saw room, employees were using a Wellsaw bandsaw that was not guarded to keep operators hands away from the cutting edges, exposing employees to cuts and amputations.

29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:  a.  On or about April 1, 2014, in the grinder room next to the detail shop, employees were using a bench grinder that was not guarded to keep operators face, body and hands away from the spinning or exploding abrasive wheel, exposing employees to cuts, bruises, and amputations.
