29 CFR 1910.217(c)(3)(iv)(d): Pullout device(s) on mechanical power press(es) were not visually inspected and checked for proper adjustment at the start of each operator shift, following a new die setup, and when operators were changed:  a)  Press 6085 - On or about March 11, 2014, the metal clamp used to hold the pullback cords in place was damaged and not adequately securing the cords to prevent them from coming loose.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:  a)  Press Room - On or about March 11, 2014, the employer has not performed inspections of the safeguards installed on mechanical power presses since 2012.
