29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    On or about 04/10/2014 at the 385 Pleasant Street Construction Project, the employees were not trained on the manufacturers' instructions and limitations of use of the personal fall arrest systems in use.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    On or about 04/10/2014 at the 385 Pleasant Street Construction Project, an employee was using a personal fall arrest system that was not rigged to prevent contact with lower levels.    The Cherry Hill Glass Company was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 315555763, citation number 02, item number 001 and was affirmed as a final order on 09/13/2011, with respect to a workplace located at 644 West Putnam Avenue in Greenwich, CT.
