29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift: a. On or about April 9, 2014, at the above addressed jobsite, an employee working from the basket of the Genie S-40 aerial lift,approximately 15 feet above the building entrance, was not connected to the basket with a lanyard.
