29 CFR 1926.1060(b):     The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training:    a) Construction site, Quad 4 stairs - on or about March 27, 2014, employees used concrete stairs with more than four risers that did not have at least one handrail for the full height of the stairs, to access their work area, exposing them to serious injury in the event of a trip or fall.

29 CFR 1926.502(b)(4): When the 200 pound (890 N) test load specified in paragraph (b)(3) of this section was applied in a downward direction, the top edge of the guardrail deflected to a height less than 39 inches (1.0 m) above the walking/working level    a) Construction site, West end Floor 2 - on or about March 27, 2014, the controlling contractor did not ensure wire rope guardrails were maintained to prevent deflection below the 39 inch minimum when a load less than 200 pounds was applied in a downward direction, exposing employees walking and working in the area to a fall hazard greater than six feet.
