29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1): (a) Shop area: On or about April 11, 2014, and at times prior, employees were exposed to shock and fire hazards when using the overhead radial arm saw that had damaged protective sheathing.

29 CFR 1910.305(g)(2)(ii): Hard-service cord and junior hard-service cord No. 14 and larger were repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced: (a) Shop area: On or about April 11, 2014, and at times prior, employees were exposed to shock and fire hazards when using the blue flexible extension cord that had significant damage to its protective sheathing. (b) Shop area: On or about April 11, 2014, and at times prior, employees were exposed to shock and fire hazards when using the yellow and black flexible extension cords that were permanently taped together.
