29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On April 16, 2014 employees were not protected from a fall while roofing a commercial roof, exposing employes to a fall of over 25 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: On or about April 16, 2014 employees were exposed to the hazardous chemicals such as TPO roof membrane cleaner and adhesive. The company did not develop a hazard communications program which includes training labeling and safety data sheets. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
