Worksite: The employee who was working from the metal frame scaffold platform at a height above 10 feet was not protected from fall hazards by the use of guardrails or a personal fall arrest system.

Worksite: The employee who was working from the metal frame scaffold that was not fully planked, and without fall protection at a height above 10 feet was not trained to recognize the hazards associated with the type of scaffold being used.
