29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Haldeman Ford, Worksite, Compressor Area on or about April 14, 2014, the electrical service conduits and panels supplying 240 volts Alternating Current for the facilitys air compressors and vehicle lifts was fully removed from the adjacent wall and then tied to a tubular welded frame scaffold in a manner that is not in accordance with its listing and labeling.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Haldeman Ford, Worksite - on or about April 14, 2014, employees were climbing the cross braces of tubular welded frame scaffolds, at heights of up to approximately 13.5 feet above grade, while conducting masonry work.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Haldeman Ford, Worksite - on or about April 14, 2014, an employee was performing masonry work on a low slope roof, approximately 17 feet above grade, without any means of fall protection. BEStucco, Inc., was previously cited for a violation of this Occupational Safety and Health Standard which was contained in OSHA Inspection Number 314727090, Citation Number 1, Item Number 2, and was affirmed as a final order on February 8, 2012, with respect to a workplace located on Grape Street, Whitehall, PA 18052.
