29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about 04/24/14 - 15, Oakshire Place Huntsville AL, employees performing roofing work were not protected from falls by the use of a fall protection system, exposing employees to falls of 16 to 25 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On or about 04/24/14 - 15 Oakshire Place, Huntsville, AL, portable extension ladder being used by employees was not extended at least 3 feet above the landings of the steep roof.
