29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a)  1110 SHELLER AVENUE, CHAMBERSBURG, PA, 17201 - Employee(s) used a JLG 600AJ aerial lift to apply stucco to the exterior of the structure, and the employer did not provide fall protection for the employee(s), thereby exposing employee(s) to a fall ranging from approximately one (1) to sixteen (16) feet to the ground below, on or about April 24, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a)  1110 SHELLER AVENUE, CHAMBERSBURG, PA 17201 - Employee(s) installed shingles on the hotel roof, and the employee(s) were not protected by any type of fall protection, thereby exposing the employee(s) to possible fall hazards of approximately eighteen (18) feet, on or about April 24, 2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  (a)  1110 SHELLER AVENUE, CHAMBERSBURG, PA, 17201 - Employees working at heights greater than six feet were not trained in the recognition of the hazards of falling and the procedures to be followed in order to minimize these hazards, thereby exposing employees to serious injury, on or about April 24, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:  (a)  1110 SHELLER AVENUE, CHAMBERSBURG, PA, 17201 - Employees used a Dewalt DW130 electric drill to mix stucco, and the power cord of the drill had been repaired with black electrical tape, thereby exposing employees to electrical shock injuries, on or about April 24, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
