29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:    a)  Bridge - On or about 04/25/2014, horizontal lifelines was used to provide employee fall protection during a bridge demolition project were not installed, moved and used by construction workers under the supervision of a qualified person.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
