The full revolution mechanical power press was activated by a foot pedal. Employees using the press to crimp cages were exposed to crushed-by hazards. b) Bay Three, 207 East Brown Street, Blairsville, Pennsylvania 15717: Observed on or about May 01, 2014, the 25 ton capacity #15 Brown Boggs (marked #13) press was not equipped with a point of operation guard or point of operation device. The full revolution mechanical power press was activated by the use of a foot pedal. Employees using the press to shear metal sleeves were exposed to crushed-by hazards.

The full revolution mechanical power press would continue to run while the foot pedal was held down. Employees using the press to crimp cages were exposed to crushed-by hazards. b) Bay One, 207 East Brown Street, Blairsville, Pennsylvania 15717: Observed on or about May 01, 2014, the 15 ton capacity #14 Bliss crimp press did not incorporate a single-stroke mechanism. The full revolution mechanical power press would continue to run while the foot pedal was held down. Employees using the press to crimp cages were exposed to crushed-by hazards. c) Bay One, 207 East Brown Street, Blairsville, Pennsylvania 15717: Observed on or about May 01, 2014, the 10 ton capacity #28 Cleve. press did not incorporate a single-stroke mechanism. The full revolution mechanical power press would continue to run while the two handed trip palm buttons were held down. Employees using the press to manufacture caps were exposed to crushed-by hazards.

The press continued to operate after the main power disconnect was turned off and then turned back on. Employees using the press to crimp cages were exposed to crushed-by hazards. b) Bay One, 207 East Brown Street, Blairsville, Pennsylvania 15717: Observed on or about May 01, 2014, the 15 ton capacity #14 Bliss crimp press did not incorporate a type of drive motor starter that would disconnect the drive motor from the power source in event of a loss of power to the press resulting from a planned or unexpected loss of power. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Bay One, 207 East Brown Street, Blairsville, Pennsylvania 15717: Observed on or about May 01, 2014, an electrical outlet box (220 Volt) was being used in a manner for which it was not designed. The outlet box was being used as a pendant style device but was equipped with knockouts on each side. The knockouts could become dislodged by an employee's fingers while plugging devices into the outlet box. The outlet box was being used to supply power to a JDS brand cyclone (model number 2100 - CKV). Employees using the outlet box were exposed to the hazard of electric shock and burns.
