29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Jobsite:  Employees did not recognize the potential hazards posed by placing too many catenary scaffolds on support cables or by failing to re-tighten wire rope clips per the manufacturers recommendations.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  Jobsite:  Bridge deck adjacent to fixed ladder at Span 15, south side of bridge  Employees had to climb onto and over an approximately 35 inch concrete barrier to climb onto and off of a fixed ladder used to access the bridge substructure.
