29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On 16 April 2014, five roofers were installing shingles on the roof of a commercial building under construction with a pneumatic nail gun without the use of eye protection, exposing the workers to the hazard of being struck by flying objects or debris.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system. On 16 April 2014, three workers were installing roofing shingles on a low sloped roof of a commercial building under construction without fall protection, exposing the workers to a fall hazard of approximately 10 feet 8 inches to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On 16 April 2014, five roofers gained access to the roof of a commercial building under construction by means of a 24 foot aluminum extension ladder that was not extended at least 3 feet beyond the roof edge, exposing the workers to a fall hazard of approximately 10 feet 8 inches to the ground below.
