29 CFR 1926.95(c): Personal protective equipment was not of safe design and construction for the work to be performed: (a) Big Al's Insulating Inc, at 2125 W 34th Avenue, Denver, CO: On and preceding 5/20/14, Big Al's Insulating Inc did not ensure that personal protective equipment was of safe design and construction for the work to be performed. Two employees sprayed Johns Manville JM Corbond (A) ISO spray foam insulation (containing 30-60% methylene bis(phenylisocyanate) and 30-60% polymethylene polyphenylene isocyanate) in a home being remodeled. Employees wore G-Tek MaxiFoam nitrile coated gloves. Nitrile coated gloves do not provide adequate protection from exposure to isocyanates. The employer did not ensure that appropriate gloves were used. This condition exposed employees to a chemical hazard.

The employer required the employees to wear 3M full face air purifying respirators with model 60923 (multigas/P100) cartridges. The employer did not provide a medical evaluation for each employee. This condition potentially exposed the employees to a respiratory hazard.
