29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) An employee was exposed to a fall hazard of up to approximately 13.4 feet while installing shingles on a roof of an apartment building without appropriate fall protection.    b) Employees were exposed to fall hazards of up to approximately 17 feet while removing shingles on roofs of an apartment complex without appropriate fall protection.

29 CFR 1926.503(a)(1):   The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees were exposed to fall hazards of approximately 12 feet while removing and installing shingles on a residential roof without appropriate fall protection and were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize the hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     a)  An employee installing shingles was exposed to a fall hazard of approximately 13.4 feet to a lower level, due to the fact that the ladder used to access the roofs did not extend three (3) feet above the upper landing surface.     b)  Employees removing shingles were exposed to fall hazards of up to approximately 17 feet to a lower level, due to the fact that the ladder used to access the roofs did not extend three (3) feet above the upper landing surface.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.    Employees were exposed to a falls of up to approximately 17 feet when using extension ladders to access the roof to perform roofing operations and the side rails of the ladder extended approximately one (1) and two (2) foot above the landing surface. The employer did not provide training for each employee that would have enabled them to recognize the hazards and provided them with the procedures to follow to minimize these hazards.
