On or about May 21, 2014, employees working within an excavation / trench were exposed to loose rock / soil located within two feet of the trench edge.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).     On or about May 21, 2014, employees working within an excavation / trench were doing so without cave-in protection.      Town & Country Underground Utilities Construction was previously cited for a violation of this or its equivalent Occupational Safety and Health Standard 29 CFR 1926.652(a)(1), which was contained in OSHA Inspection Number 313138364, Citation Number 1, Item Number 1, Issued on 9/1/2011, and was affirmed final order on 5/17/2012, with respect to a workplace located at Bridge Rd & Henniger St, Mayville, WI 53050.      Town & Country Underground Utilities Const, Inc. was previously cited for a violation of this or its equivalent Occupational Safety and Health Standard 29 CFR 1926.652(a)(1), which was contained in OSHA Inspection Number 313524365, Citation Number 2, Item Number 1, Issued on 1/6/2010, and was affirmed final order on 1/29/2010, with respect to a workplace located at N115 W16560, Germantown, WI 53022.
