29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced:  (a)  Warehouse:  On or about May 21, 2014 and times prior thereto a welding cable available for use had a cut to the conductor.  This exposed employees to potential shock/burn hazards if they contacted the conductor.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.254.(d)(9)(iii), which was contained in OSHA inspection number 898234, notice number 1, item number 2 and was affirmed as a final order on July 1, 2013 , with respect to a workplace located at Sweet Home Ranger District.
