Potential fall height was measured to be 21' from eave to ground with a roof pitch of 6/12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: -The employees were using a Werner aluminum extension ladder for access and egress to and from the roof. The side rails of the ladder did not extend 36" above the upper landing surface to allow for a hand hold for employees.
