29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: Reforestation TSI: On or about May 20th, 2014 and times prior thereto, the ladder way did not have a swinging gate or offset to protect employees. This exposed employees to a potential fall of 10 feet to the floor. The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 106753, notice number 2, item number 1 and was affirmed as a final order on 11/28/2011, with respect to a workplace located at Pine Ranger Station in Halfway, OR.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: (a) Auto shop: On or about May 20th, 2014 and times prior thereto, there was an air nozzle that would not reduce to below 30 p.s.i. This potentially exposed employees to potential air embolism/laceration hazards. (b) Engine Bay: On or about May 20th, 2014 and times prior thereto, there was an air nozzle that would not reduce to below 30 p.s.i. This potentially exposed employees to potential air embolism/laceration hazards. The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 903923, notice number 1, item number 5 and was affirmed as a final order on 07/01/2013, with respect to a workplace located at the Tiller Ranger District in Roseburg, Oregon.
