29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  (a) On or about May 20, 2014 the inspection revealed a Skilsaw model HD77 that was available for use and had damaged outer insulation. The Skilsaw was located in the office adjacent to the wood shop at the Sisters Ranger District compound.
