This condition exposed employees to struck-by eye hazards.   b) Gullet Countracting, LLC @ 3532 St. Albans Rd., St. Albans, MO 63073: On May 23, 2014, the employer did not ensure that employees were protected from flying debris while removing roofing materials while conducting residential roofing tear-off work. This condition exposed employees to struck-by eye hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:     a) Gullet Contracting, LLC @ 3532 St. Albans Rd., St. Albans, MO 63073: On May 23, 2014, the employer did not ensure that employees were protected against fall hazards. Three employees conducted roofing work on a two-story, residential home without the use of a guardrails or personal fall arrest systems. Employees conducted roofing work on a 10 in 12 pitch portion of the roof. This condition exposed employees to a fall hazard of approximately 20 feet.   b) Gullet Contracting, LLC @ 3532 St. Albans Rd., St. Albans, MO 63073: On May 23, 2014, the employer did not ensure that employees were protected against fall hazards. Two employees conducted roofing work on a two-story, residential home without the use of a guardrails or personal fall arrest systems. Employees conducted roofing work on a 4 in 12 pitch portion of the roof. This condition exposed employees to a fall hazard of approximately 12 feet.
