OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from an unprotected PVC pipe: (a) From the Warehouse area to the Screening pump room: Employees of AAA Sign & Safety Products were exposed to the hazards of being struck by fragments of plastic pipe (PVC 1120 Schedule 40). The pipe was used to transport compressed air from the warehouse area to operate the screening pump machine. The piping ran from the 60 gallon air compressor and along the top of the wall from into the screening pump room. The compressed air was measured at 60 pounds per square inch (PSI).

A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. (a) Rear exit door of the warehouse: The exit door was locked from the inside by a pry bar and the employees were exposed to an entrapment hazard.
