OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from an unprotected PVC pipe:  (a) From the Warehouse area to the Screening pump room: Employees of AAA Sign & Safety Products were exposed to the hazards of being struck by fragments of plastic pipe (PVC 1120 Schedule 40). The pipe was used to transport compressed air from the warehouse area to operate the screening pump machine. The piping ran from the 60 gallon air compressor and along the top of the wall from into the screening pump room. The compressed air was measured at 60 pounds per square inch (PSI).

29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.  (a) Rear exit door of the warehouse: The exit door was locked from the inside by a pry bar and the employees were exposed to an entrapment hazard.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   (a) Northern Industries band saw: The employer did not ensure that the horizontal band saw belt was guarded, exposing employees to amputation hazards from rotating parts.  (b) Wilton band saw: The employer did not ensure that the horizontal band saw belt was guarded, exposing employees to amputation hazards from rotating parts.

29 CFR 1910.303(f)(1): Motors and appliances. Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  (a) Electrical panel located toward the front of the screening room: The employer did not ensure that disconnects in the electrical panel were marked and labeled with information indicating it's purpose, which controlled power to the lights in the main building.

When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  (a) Electrical breaker panel located toward the front of the screening room: The employer was using the space at the electrical breaker panel to store print screen frames and it was not accessible. The area was blocked with wood printing screen frames.
