29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  (a)  40 Dauphin Drive, Mechanicsburg Pennsylvania - Employees operated powered industrial truck to move products.  The employer did not ensure that employees had successfully completed powered industrial truck training prior to permitting an employee to operate a powered industrial truck, on or about June 9, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(3)(i)(A): Powered industrial truck operators did not receive training on operating instructions, warnings, and precautions for the types of truck the operator was authorized to operate:  (a) 40 Dauphin Drive, Mechanicsburg Pennsylvania - Employees operated powered industrial trucks to move products.  The employer did not ensure that employees had an understanding of  and the warnings or precautions necessary to protect themselves from the under ride hazard, on or about June 9, 2014.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
