29 CFR 1926.105(a): Safety nets were not provided when workplaces were more than 25 feet above the ground or water surface, or other surface(s) where the use of ladders, scaffolds, catch platforms, temporary floors, safety lines, or safety belts was impractical: (a). Worksite located FCC Site 1287947, Adrian, Missouri- Employees were exposed to fall hazards and were not adequately protected from falling by the use of safety nets or personal fall arrest systems. The employees were provided with fall arrest systems, but were not utilized at all times and was not a protective system in that, the pelican hooks were too large for the step-bolt anchors being used by some of the employees while ascending and descending.

29 CFR 1926.1053(a)(8): A metal spreader or locking device shall be provided on each stepladder to hold the front and back sections in an open position when the ladder is being used: (a). Worksite located at FCC Site 1287947, Adrian, Missouri- Employees were exposed to fall hazards in that a step-ladder was leaned up against the tower legs without the metal spreader bar extended.

Worksite located at FCC Site 1287947, Adrian, Missouri- Employees were exposed to a fall hazard in that the top steps of the step ladder that was leaning against the tower legs were being used by the employees.
