29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:   On or about June 5,2014 at the EXLTUBE facility- Employees were exposed to an industrial fan that was less than 7 feet above the floor or working level and was missing the front guard.    Abatement certification for this item is not required.

29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less.   On or about June 5, 2014 at the EXLTUBE facility- Employees were exposed to a blocked 480V electrical panel that did not have a sufficient entrance that gave access to the working space.    Abatement certification for this item is not required.

29 CFR 1910.305(d)(2):   Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.  On or about June 5, 2014 at the EXLTUBE facility- employees in the job shop were exposed to hazard of electrocution from open areas within an electrical panel which did not provide a protection from inadvertent contact with exposed live electrical connections.   Abatement certification for this item is not required.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:     On or about June 5, 2014 at the EXLTUBE facility- employees were exposed to a 120V flexible cable had pulled loose from its attachment point to a portable outlet box leaving the cable attached to the device by its internal conductors.   Abatement certification is not required for this item.
