This condition exposed employees to electrical shock and electrical burn hazard.

29 CFR 1917.151(g)(4): Work rests on fixed grinding machines were not adjusted closely to the wheel with a maximum opening of 1/8-inch (3.18 mm) and securely clamped.   Mechanics shop; on or about 12 June 2014 and times prior thereto; the work rest on a Dayton Commercial bench grinder, serial number unreadable, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured 1/4 of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1917.111(b): Maximum safe load limits, in pounds per square foot (kilograms per square meter), of floors elevated above ground level, and pier structures over the water were not conspicuously posted in all cargo areas:  (a) Main Ramp: On or about June 12, 2014 and at times prior thereto, maximum safe load limits had not been posted.  This condition exposed the ramp to potential damage from the transferring of overloaded cargo across the ramp.  (b) Secondary Ramp: On or about June 12, 2014 and at times prior thereto, maximum safe load limits had not been posted.  This condition exposed the ramp to potential damage from the transferring of overloaded cargo across the ramp.
