29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Production departments, thread rolling and cold heading machines: employees were exposed to amputation hazards during servicing, changeovers, and other setup tasks where no energy control program was established and implemented in the workplace.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed:    Production departments, thread rolling and cold heading machines: employees performing setup and changeovers were exposed to caught-in between hands and fingers amputation hazards when procedures for machines testing or positioning (setup) in the point-of-operation and associated danger zones were not established in a sequence of actions required to maintain the integrity and continuity of employee protection to clear machines of tools and materials; remove employees from the hazardous areas around the machine; remove the lockout/tagout devices; energize the machine and employ effective employee protection while testing or positioning machinery; and, turn off all systems, isolate the machine from the energy source, and reapply lockout/tagout devices as specified, if additional servicing or maintenance is required.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

(b) Heading department, cold heading machines #3, #22, #23, #24, #25, #26, #27, #28, #34, #21 #70: operators were exposed to caught-in between fingers and hands amputation hazards where adequate point-of-operation guards and/or safeguarding devices were not provided, or were by-passed in a manner that rendered them ineffective as constructed and designed to prevent access to the danger zone.    (c) Tool room, a hydraulic press: operators were exposed to caught-in between fingers and hands amputation hazards where a point-of-operation safeguarding device was by-passed in a manner that rendered it ineffective as constructed and designed to prevent access to the danger zone. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Heading department, header machine #26 AOT-12-B, yellow extension cord E53440: on June 19, 2014, employees were exposed to electric shock hazard where listed and labeled electrical equipment was not installed and used in accordance with the manufacturer's provisions and other applicable listing standards for that type of equipment. Energized flexible extension cord was powering multiple equipment through connection to a "Y" splitter power cord, and was used as a substitute for fixed wiring of a structure where wrapped around overhead ducting pipes.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    (b) Heading department, header machine #27 AOT-12-B, unmarked yellow and black flexible extension cords: on June 19, 2014, employees were exposed to electric shock hazards where listed and labeled electrical equipment was not installed and used in accordance with the manufacturers' provisions and other applicable listing standards for that type of equipment. Energized flexible extension cords were wrapped with black tape at their plug connections to each other, and were used as a substitute for fixed wiring of a structure.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    (c) Heading department, parts' quality check stations: employees were exposed to electric shock hazards where household use only light bulbs were exposed to physical damage.
