29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:  At this job site a trench shield (trench box) was installed such that there was a gap of approximately two and one-half to three feet between the trench shield walls and the walls of the trench on both sides of the trench. Employees installing a new eight inch sewer line were exposed to being caught between the sewer line and the trench box in the event of sudden movement of the surrounding dirt walls.
