29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws::                                                                   5202 L St., Omaha, NE 68107 (Job-Site): The employer did not ensure that a flexible cord on a power sprayer had proper strain relief.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citation.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge  which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      5202 L St., Omaha, NE 68107 (Job-site)  The employer is failing to ensure that adequate fall protection was utilized by An employee was working on overhang platform that was over 9 feet from the ground.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
