29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  On or about May 9, 2014 at 125 Neeleytown Road, Montgomery NY, in the truck repair facility, a procedure to control potentially hazardous energy was not available or developed for servicing the Graco grease pump.  Employees were exposed to a amputation hazard when the compressed air in the grease pump was not correctly neutralized prior to working on the pump due to the lack of the procedure.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    a)  On or about May 9, 2014 at 125 Neeleytown Road, Montogmery, NY 12549 employees were not trained on a specific procedure to neutralize the stored energy in the Graco grease pump.  The specific procedure to control the residual energy (compressed air) in the grease pump was not developed and accordingly the training was needed on a specific procedure yet to be developed.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a)  On or about June 20, 2014 at 125 Neeleytown Road, Montgomery, NY 12549 a fire extinguisher was not mounted and identified by a sign but was sitting on the floor of bay 1 near the wall closest to the service desk.
