29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, personnel hoist, or similar surface was not used:    a. On or about June 12, 2014,  there was no safe ladder access for employees working from a scaffold system, exposing the employee to a 13 feet fall hazard.

On or about June 12, 2014, the employee working from a carpenter's scaffold system was not protected from falling approximately 13 feet to the ground below by the use of a guardrail system, safety net system, personal fall arrest system or any other type fall protection method.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a. On or about June 12, 2014, employees accessing to second floor by way of a walk board were exposed to a fall hazard of 9.5 feet because there were no guardrail system, safety net system, personal fall arrest system or any other type fall protection method was being used.
