29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about June 26, 2014, and at times prior thereto, the employer did not develop or implement written lockout/tagout procedures for each type of equipment or machine located within the plant. Employees performing maintenance work on the equipment such as, but not limited to the Smetco machines were exposed to electrical shock, caught-between and struck-by hazards.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:   On or about June 26, 2014, and at times prior thereto, the employer did ensure that stacks of pallets located throughout the production area were stacked, blocked, interlocked or limited in height so they were stable and secure against sliding and collapse. Employees were exposed to being crushed by falling pallets.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a).   On or about August 18, 2014,  in the production area, employees were using a Dewalt hand held grinder without the safety guard, exposing employees to laceration hazards.
