A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: Along the south side of the construction site, on or about July 02, 2014, an employee was exposed to a cave-in hazard while working inside a trench approximately 6 feet deep by 4 feet wide by 60 feet long without having a safe means of egress.
