Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and/or amputation hazards:     (a) On or about June 21, 2014, for the employee who suffered finger amputations while operating power tongs during casing installation at a Zavanna LLC well site, Angus 3-10 5H, LOT4 3-153-99, Williston, ND.     Abatement Note: Among other methods, one feasible and acceptable means of abatement to correct the hazard would be for the employer to ensure the power tong doors are in proper working order and provide employee training and instruction needed to safely perform the specific work activity(ies) and in accordance with (API) American Petroleum Institute Recommended Practice 54, Third Edition (Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations), Section 9.9, DRILL STRING HANDLING EQUIPMENT, Subsection 9.9.8 and Section 1.3, RESPONSIBILITIES.
